Whilst you are asked to reflect on your supporting information, you are NOT expected to use every template offered - only those you wish to use and that are applicable to your needs (you may have documented your reflections elsewhere in the appraisal forms).
Remember, the templates are there to aid you as additional supporting information, they are NOT mandatory.
Right-click on the document links below and save the file to your computer first. Once the template has been saved you should be able to navigate to the folder you saved it to, and open it. Once you have completed your reflections, save the file (in original file extension) and upload it as part of your supporting information.
Submission of a CPD log is required every year as part of your appraisal.
If you use an existing College or other CPD system or log, please upload an export of this instead as part of your supporting information and detail the main learning points for discussion with your appraiser.
SOAR provides an online CPD log for your use. An offline Excel version of this (download below) is available if you so wish.
Regardless of which CPD log you use, please ensure you reflect on your learning. You can find further information regarding CPD on the GMC website.
