Form 5A - for exemptions from appraisal - is used in lieu of a Form 4 to complete an appraisal e.g. mat leave, sick leave, sabbatical etc.
Form 5A can be created by either the appraiser or yourselves - the health board admin team (for instance, Form 5A was used by some boards during the Covid-19 pandemic and this was actioned by admin teams.)
To create a Form 5A:
- Create a dummy appraisal (see item 8 above), set the date to midnight on 31st March and set the corresponding appraisal period - click Save
- You should see a button on the right to "Create Form 5A" - click on this
- You will be prompted to make sure you've discussed it with your Appraisal Lead first - click OK
- In the top of the Form 5A select the Appraisal Lead you need to sign off the form
- Complete the rest of the form as needed
- When finished, click on the "Submit" button at bottom of the page to send to selected Appraisal Lead for their review and sign off.
Check for incomplete Form 5A
To check on whose Form 5A's still to be signed off, you can search the appraisal using both the interview and the form statuses:

- Click on "Interviews" from the menu
- in the "Interview status" dropdown select "To be submitted and in progress"
- in the "Form status" dropdown select "Form 5A - Draft"
- Click "Search"
- Repeat steps 3 and 4 but select "Form 5A - Forwarded"
This will bring up a list of appraisees whose appraisal are not completed and has a Form 5A in progress.
If the Form 5A is in draft, you will need to liaise with the appraiser.
If the Form 5A is forwarded, it is waiting for the Appraisal Lead to sign off.
