Domain 1: Knowledge, Skills & Performance


Examples of supporting information include course certificates, and a CPD logbook.  If you do not use one, an example learning log spreadsheet is available for download at the bottom of this page.

Knowledge, Skills & Performance

On the online appraisal form (from SOAR) in Domain 1, the first two parts of Domain 1 are standard for all GPs.  You are asked to:

  • Review last year's learning
  • Propose a learning plan for the year ahead

Use the links in the form to guide you through this. You will also find reflective templates relating to your learning activities through these links.  The example learning log spreadsheet offered allows you to record your learning and count credits if you wish to use it.

The Performance element of this domain allows you to present and reflect on aspects of the quality of the work you do.

This is different from the Quality Improvement Activities in Domain 2 (audit, etc) and it is intended as a place for describing or demonstrating some of the ways in which you try to ensure a high standard of work, and areas that you see as a problem, for reflective discussion with your appraiser.

There are no essential pieces of supporting information for this.

Last year's learning and your new PDP

Sessional GPs should use the main toolkit for this as there are no specific differences for all GPs.

Quality of the work that you do

A number of specific tools, suitable for sessional doctors, is used to provide supporting information under this heading:

  • Drugs availability
  • Equipment availability
  • Drug stock management spreadsheet

If you have undertaken a long term locum, record your reflections on an area in which you held the lead.

The following is listed in Domain 2 but could also be used in this domain:

  • Reflective case studies that you have discussed in a small study group (such as PBSGL)

The following could be used in this Domain but are listed under Domain 3:

  • Induction and preparation for work as a locum
  • Patient enablement questionnaire
  • Formative observation proforma
  • Video or audio records - record of review with/by trained colleague

We suggest that you can also use the "Quality of Your Work" reflective template from the main Toolkit to reflect on aspects of what you do as a sessional GP (listed below).

Learning Log sample (macro-less version)

An online CPD log has been added to SOAR so you can enter your CPD entries directly, rather than using this spreadsheet. However, if you wish to use the spreadsheet instead, please note the following instructions: This Excel spreadsheet is designed to help the user record their Learning Activities throughout the year. Save this spreadsheet to your computer (right-click > Save target as) and work on this as you go throughout the year. Upload the final version to SOAR when the time comes for your appraisal. Make sure you check the worksheet "Examples and Tips" for guidance. Although originally designed specifically for GPs, it can also be used by consultants (just ignore the impact scoring).
